How to Donate?
如何捐款？
You can help us in the following ways:
您可通过以下途径协助我们
1) Visit Singa Care Medical personally to make your donation towards our Medical Fund.亲临恒顺慈善医疗中心捐助医药基金
2) Download the Donation Form, after completion the particulars
下载捐款表格，填妥个人或公司资料后,
a) For Credit Card, fill in all the necessary items, sign the form, e-mail / fax or post the completed form to Singa Care Medical.
如用信用卡捐款，请将填妥表格邮寄，传真或电邮至本中心。
b) For Cheques, together with the crossed cheque, make payable to "Singa Care Medical" and Post both items to Singa Care Medical in an enclosed envelope.
如用支票，捐款表格联同划线支票，写付“Singa Care Medical”邮寄来本中心。
3) For GIRO, download the GIRO Form, fill in all the necessary items, sign the form, e-mail / fax or post the completed form to Singa Care Medical.
如用财路捐款，请将填妥表格邮寄，传真或电邮至本中心。
3) Organizations or individuals who would like to place our donation boxes at your premises to allow public to make donation to our Society are most welcome to contact us at Tel: 6291 6609
商家或热心人士有意置放本院乐捐箱于贵商号，任由公众随缘乐捐，可拨电与本中心联络 6291 6609。
